Incident Reporting & Communication Policy

Purpose:

Our goal is to make sure that all concerns, incidents, and questions are handled quickly, clearly, and respectfully. This policy explains how to report issues, who to contact, and how we work together to keep AJFC a safe and positive environment for all athletes and families.

This policy applies to any incidents that happen on AJFC property or at league-sponsored events.


1. Reporting Incidents

If something happens—an injury, safety concern, behavior issue, or property damage—we ask that it be reported as soon as possible. Ideally, incidents should be reported within 48 hours so we can respond quickly and appropriately.

How to Report an Incident:

• Email help@ajfc.org within 48 hours.

• Please include:

– Date, time, and location

– Names of those involved

– A clear description of what happened

– Any steps taken at the time (first aid, contacting emergency services, etc.)

Timely reporting helps us take the right steps to support everyone involved.


2. Immediate Response – Urgent Issues Only

If something requires immediate attention—such as a safety emergency or a situation that cannot wait—please contact the President directly.

3. Non-Emergent Questions or Concerns

For general questions or concerns (scheduling, team questions, non-urgent issues), please reach out to the appropriate program contacts below. This helps ensure that concerns are handled quickly without going through emergency channels.

4. Anonymous Complaints

For us to properly address and follow up on concerns, we do not accept anonymous incident reports. Open communication helps us ensure fairness, accuracy, and accountability. Families are still welcome to share general feedback through our anonymous end-of-season survey.


5. Why Timely Reporting Matters

• Timeliness: Reporting within 48 hours helps us gather accurate information and address the situation quickly.

• Accuracy: Clear details help prevent misunderstandings.

• Accountability: Following the reporting process ensures all concerns are handled consistently and

respectfully.


6. Confidentiality

All incident reports are handled professionally and confidentially. Only those who need to be involved in

resolving the issue will be informed.
